 FOR THOSE SENDING IN VIDEOS AND THOSE BEING VIDEO TAPED OR PHOTOGRAPHED.

1) I hereby authorize Godz Peepz and its representative to photograph me, take motion pictures of me, take video footage of me, and/or make electronic sound recordings of me (herein referred to as photographic or electronic reproductions).

2) I authorize the use of any such photographic or electronic reproductions of me for any purpose, including, but not limited to Internet TV and other public media as may be deemed appropriate by Godz Peepz and its representative (I understand that I may be identifiable from such photographic or electronic reproduction)

3) I understand and accept that I/we will not receive any compensation of any kind whatsoever indefinitely for supplying my photograph, motion pictures, video footage, and/or electronic sound recordings (herein referred to as photographic or electronic reproductions).
